OSHA has cited a demolition and concrete recycling company in Newark with 11 serious safety violations. The New Jersey company uses two names: T. Fiore Demolition Inc. and T. Fiore Recycling Corp.
Good practices for confined space and lockout/tagout

To ensure that California employers do everything they can to prevent serious injury or death as a result of working in a confined space, The Phylmar Regulatory Roundtable (PRR) -- a group of companies committed to improving workplace safety and health -- compiled information from members companies existing safety and health programs and created materials that provide the public with a set of successful or good practices for confined space and lockout/tagout procedures for those who do not have these resources available to them.
An OSHA inspection begun after a worker was found dead inside a rotating drum assembly at Refuse Recycling in Marietta, Ohio, has resulted in 21 safety citations against Marietta Industrial Enterprises, Inc., which operates the facility.
An OSHA investigation undertaken after a maintenance employee at Anchor Hocking in Lancaster, Ohio suffered an amputation found that workers had not been trained in recommended “lockout/tagout” procedures for isolating the energy sources of machines to prevent their accidental operation.
