A Phenix City, Alabama insulation manufacturer initially corrected hazards that included failing to protect its workers from caustic chemicals, but did not maintain those corrections, according to OSHA inspectors, who visited the facility after receiving a complaint about dangerous conditions.
Industrial Insulation Group LLC was cited for five serious safety and health violations following that inspection. Proposed penalties total $41,800.
