A California construction company that dismantled a trench box while an employee was still working inside, causing him to be fatally crushed, has been cited by Cal/OSHA for safety violations. The agency determined that general contractor Bay Construction Co. committed willful-serious safety violations by unsafely removing a linear support rail that fell and killed the worker.

An employee of St. Charles/Resurrection Cemeteries in Farmingdale, New York, was seriously injured on May 7, 2015, when the walls of the grave opening in which he was working collapsed and buried him up to his waist.

An Auburn, Ala.-based contractor exposed workers to dangerous cave-in hazards and failed to use safety measures to prevent excavation collapse, OSHA inspectors found, in an investigation conducted as part of the agency’s National Emphasis Program on Trenching and Excavation.

Despite being cited by OSHA following a 2013 trench collapse, a follow-up inspection found that C & G Refrigeration Inc. of Hanover, Pennsylvania, was still exposing its employees to trenching hazards to potentially deadly trenching hazards while they performed underground utility work at a residence.
