FedEx worker dies in accident at Indianapolis airport

Indianapolis, IN — A FedEx employee died Thursday night in an accident at the Indianapolis International Airport. FedEx reported the workplace fatality Friday morning, Stephanie McFarland, a spokeswoman for the Indiana Department of Labor and its Indiana Occupational Safety and Health Administration, said.

Man killed by bulldozer at construction site

SCITUATE, MASS. — Workplace safety officials are investigating the death of a 61-year-old man who was struck by a bulldozer at a construction site in Scituate. Authorities say the man was dead when police and paramedics arrived at the scene Friday afternoon. His name has not been released.

Google employee, 22, found dead inside NYC Headquarters

Chelsea, NY — A “vibrant” Google software engineer was found dead inside the tech giant’s Chelsea headquarters, according to cops and sources. A janitor found 22-year-old Scott Krulcik unconscious at his work terminal on the sixth floor of the building on Eighth Avenue near West 16th Street around 9 p.m. Friday, police sources said. EMS workers tried to perform CPR, but to no avail. Krulcik was pronounced dead at the scene. His body did not show any signs ofC trauma, and there did not appear to be criminality involved, authorities said. The city Medical Examiner’s office will determine the cause of death.

Worker killed on Rt. 33 in Union County

Jerome Township, OH — A construction worker was struck and killed this morning on U.S. 33 near State Route 161 in Union County. Teresa M. Howell, of Greenwich, Ohio, of Greenwich, Ohio, was employed by Lake Erie Construction Co., a subcontractor for the Ohio Department of Transportation. According to a release from the Union County Sheriff’s Office, deputies and units from the Jerome Township Fire Department and the Washington Township Fire Department responded to the scene and found a twenty-four-year-old female construction worker who had been struck by a tractor trailer while working in the construction zone. The victim was pronounced dead at the scene by the Union County Coroner’s Office. She was employed by the Lake Erie Construction Company.

Worker killed in accident at south Arkansas lumber company

Little Rock, AR —  A 47-year-old mechanic was killed in an accident at a lumber company in south Arkansas, authorities said. The Ouachita County sheriff’s office said Samuel Buchanan, 47, of Camden was pronounced dead after being found unresponsive under the bucket of a loader Thursday morning at the Victory Lumber Company, Ouachita County Road 67. Employees were able to use other equipment to lift the loader off of Buchanan and fire officials provided medical treatment at the scene. Buchanan was then taken to a local hospital, where he was pronounced dead.

Man dies after getting crushed between industrial beam, granite machine

ORLANDO, Fla. – A man is dead after getting crushed between an industrial beam and a large machine Thursday morning, according to officials from Orange County Fire Rescue. Officials said the 44-year-old man was working at Majestic Marble and Granite on 4948 S Orange Ave. in Orlando. Officials from the Edgewood Police Department have not yet released the victim’s name, but did say he had been working at the business for three weeks. The man was cleaning a machine that uses water to polish granite and marble when he got caught between the machine and the beam, according to fire rescue officials. They said as he was caught, the machine kept moving back and forth as it polished the marble. Eventually, the victim fell into a trench below with what co-workers thought was an arm injury, according to rescuers. They said the victim was conscious at first but began to lose his pulse, prompting his co-workers to perform CPR. Rescuers said the man went into cardiac arrest and died at Orlando Regional Medical Center.

Worcester firefighter dies while battling huge blaze, the latest in a string of firefighter deaths in city

Worcester, Ma — A Worcester, Massachusetts, firefighter died early Sunday while battling a massive fire in a 3-story building, officials said — the latest in a sad history of firefighter deaths in the city. Firefighters responded to a fire at 7 Lowell St. in Worcester just before 4 a.m., according to fire officials. When conditions deteriorated and the fire went up to five alarms, five firefighters were forced to escape the blaze using ladders, officials said during a news conference Sunday. Two firefighters were taken to the hospital, but Christopher Roy, 36, succumbed to his injuries, officials said.

Dubuque man dies after industrial accident

Dubuque, IA — A Dubuque man has died, after an industrial accident. According to the Telegraph Herald, emergency crews were called to AY McDonald Thursday morning, on a report of someone being trapped in a confined space.The Dubuque Fire Chief told the Telegraph Herald the man was taken to a hospital, but later died. The victim’s name hasn’t been released

Employee charged after owner, another employee are killed at Memphis tire shop

MEMPHIS, Tenn. — Two men are dead after a shooting at a tire shop in the 3000 block of South Third in southwest Memphis on Thursday, Memphis Police said. The victims – Kamel Al Abes and Marcus Anderson – were located with gunshot wounds and were pronounced dead at the scene. Karen Rudolph, with MPD, confirmed the victims are the owner and an employee. Police say another employee, identified as 36-year-old Ricky Hull, has been charged with two counts of first-degree murder. A motive for the shooting was not released.

Truck driver killed in Jones County rollover wreck

JONES COUNTY, Texas  – A truck driver was killed during a rollover wreck in Jones County Thursday morning. Terry Joe Mathis 69, of Rule, was pronounced dead at the scene of the crash on FM 2834 six miles northeast of  Hamlin around 10:17 a.m., according to a press release from the Texas Department of Public Safety. The press release claims Mathis was traveling west down the road when his truck experienced an axle malfunction, which caused the rig to roll. 

Greenwich woman ID’s as Lake Erie Construction worker killed on highway

Jerome Township, Ohio — Authorities continue to investigate a fatal crash involving a construction worker that occurred just before 9 a.m. today on U.S. 33 in Union County’s Jerome Township. The victim has been identified as Teresa M. Howell, 24 of Greenwich, the Union County Sheriff’s Office reported. The semi that allegedly struck Howell is a 2004 Kenworth truck that was hauling a 2014 Great Dane trailer. The company that owns the tractor and trailer is Estep Express Incorporated located in Mansfield, Ohio. 

Man dies in workplace accident in Clinton County

GREENE TOWNSHIP, Pa. — A worker died in a workplace accident Tuesday in Clinton County. The coroner says a 32-year-old man from Sunbury was hit by a large forklift at Nicholas Meats just outside Loganton and died from a head injury. His name was not released.

Oregon man dies in accident at logging site

WINSTON, Ore. (AP) – Authorities say a 57-year-old man died in an industrial accident at a logging site south of Eugene. The World reports someone called 911 at about 9:30 a.m. Tuesday requesting medical help for a man who was injured while working on a piece of equipment. Richard Allen Smith of Tenmile died at the scene. The Douglas County Medical Examiner’s Office is investigating the death.

Man killed in forklift accident

Maricopa, CA — A Porterville man was killed in an industrial accident on a farm east of Maricopa. Ubaldo Vargas Vera, 51, died at the scene of the accident on the 12000 block of Highway 166 Monday at about 10 a.m, the Kern County Sheriff-Coroner said. Vera was the operator of a forklift at the above location and was crushed by the forklift, the Sheriff-Coroner said.
No further details about the accident were released.

Roofer killed in fall

Vancouver, WA — The victim, Jesse Flores,  an employee of Roof Rabbit, was cleaning gutters when he was discovered on the ground approximately 15 feet away from where his ladder was set up. The Clark County Medical Examiner’s preliminary report notes the victim suffered a brain injury on which caused a  hemorrhage and ultimately resulted in his death two days later. Police identify dump truck driver killed in Md. crash

Washington DC — Police identified the man killed after his dump truck crashed into a tree on Monday as Albert Clark Jr., 47. Clark, of Northeast Washington, was driving east on Cedarville Road, near the 10500 block in Brandywine, about 9:30 a.m. when he lost control and the truck left the road, hitting a tree, Prince George’s County police said. Authorities pronounced Clark dead at the scene. Investigators are working to determine the cause of the crash.

Kentucky State Police: Man dies in logging accident

HARLAN, KY. (AP) — Kentucky State Police say a man died in a logging accident. News outlets report troopers were called to the Cranks community of Harlan County around 10 a.m. Tuesday morning for reports of a workplace accident. A news release says 36-year-old John Williams was injured while working. Preliminary investigation indicates that he was at working at Three States Coal Sales and Logging Company. Williams was taken to an awaiting ambulance and was pronounced dead.

Man in his 30s dies at Roslindale construction site

Roslindale, MA — Authorities are investigating the death of a man in his 30s at a Roslindale construction site, police said. Boston police were called to 1086 Canterbury St. around 7:30 a.m. Monday for a man who was unresponsive, according to a department spokesman. The victim was pronounced dead at the scene. His name wasn’t released. Additional information about the cause of death wasn’t immediately available.

Metro-North employee injured in explosion dies

WHITE PLAINS, NY — A White Plains resident and Metro-North employee died Nov. 24 after being injured in an explosion in a train yard parking lot in October. Gerald Schroeder, 67, and another employee were injured when one of their cars exploded. The railroad cannot confirm whether the death was related to the incident in October, the Journal News said, but his obituary said he died from injuries sustained in the explosion.

G. Alan Shelly dies in warehouse accident

CHAMBERSBURG, PA — Glenn Alan Shelly is the seventh person to die on the job in Franklin County this year, the deadliest in recent memory for local workplace fatalities. Shelly of Mercersburg died on Nov. 28 while working at a distribution warehouse in Guilford Township. He was 64. Emergency personnel responding to the 2:50 a.m. call on Nov. 28 at Franklin Storage found a person underneath an 1,800-pound stack of plywood, according to a report from Franklin Fire Company. The person died before rescuers arrived. They used air bags to free the victim. Shelly died of blunt force trauma to the neck and chest, according to Franklin County Coroner Jeffrey Conner.

Truck driver killed in Garvin County crash

GARVIN COUNTY, Okla. — A Texas truck driver was killed Monday morning in a fiery crash involving two 18-wheelers on Interstate 35 near Wynnewood. The Oklahoma Highway Patrol said a big rig driven by a man identified as 61-year-old Carlos A. Escoto was headed northbound on the freeway when he slammed into the back of another truck that was stopped on the shoulder near the intersection with State Highway 29. Both trucks erupted in flames. Escoto, from Garland, Texas, was pronounced dead at the scene. The driver of the other vehicle, Christopher J. McCaleb, 24, of Mesquite Texas, was taken to Mercy Hospital in Ardmore for treatment and later released.

30-Year veteran postal worker hit by car, killed on the job

Commerce, CA — A postal worker, who had been on the job for 30 years, was struck by a vehicle and killed Monday morning in Commerce. The victim identified as Joel Perales, 58, by NBC4 was standing outside a U.S. Postal Service truck about 10 a.m. in the 2100 block of South Atlantic Boulevard when a car struck him, causing fatal injuries, according to Lt. Rick Mejia of the Los Angeles County Sheriff’s Department’s East Los Angeles Station. After hitting the postal worker, the car slammed into the postal truck and then struck and came to rest against the wall of a nearby business, Mejia said. The victim was pronounced dead at a hospital.

Report details efforts to rescue mother fatally pinned by hay bale

HENDRUM, Minn. — The Norman County Sheriff’s Office has released more details of a farm accident last month that fatally injured a 28-year-old mother. Katherine Brommenschenkel Vilmo, of Ada, was feeding cattle Friday, Nov. 16, on her family’s farm in Hendrum along Minnesota Highway 200. It was about 9 a.m. when a large hay bale pinned Vilmo against a cattle feeder, according to a sheriff’s office report. Vilmo’s mother, Diane Brommenschenkel, told Sheriff Jeremy Thornton that her daughter was outside doing chores when the mother got a call from her daughter. When Brommenschenkel answered the phone, however, “there was nothing there,” the report stated. Brommenschenkel looked outside and didn’t see any movement, so she went outside to check, the report said. She told the sheriff that she found her daughter unresponsive, with a hay bale pinning her against the rail of the cattle feeder.

Man killed in construction accident in Santa Barbara

Santa Barbara, CA — A worker was killed Monday morning in a construction accident in Santa Barbara. According to police, the man was in his mid-30s. The accident happened at about 6:50 a.m. in the 600 block of East Montecito Street. Police say the man was trying to manipulate a 7,200 pound I-beam when the beam rolled back on him and crushed him. The man was pronounced dead at the scene. Cal/OSHA, the state’s Division of Occupational Safety and Health, is investigating the incident.
