NTSB rules driver fatigue probable cause of Utah motorcoach accident (4/27)

San Joaquin County District Attorney filed Involuntary Manslaughter charges and California Labor Code violations against Merced Farm Labor in the May 2008 death of 17-year-old farm worker Maria Isabel Vasquez Jimenez, according to a press release from the California Department of Industrial Relations (DIR).

OSHA proposes $66K+ in fines against Massachusetts food packager after worker injured by machinery (4/24)

OSHA has cited Brady Enterprises Inc. of Weymouth, Mass., for alleged willful, repeat and serious violations of safety and health standards at its 45 Finnell Drive plant, according to an agency press release. The food products packager faces $66,500 in proposed fines, following a Nov. 14 accident in which an employee was caught in a labeling machine while cleaning it.

Déjà vu: Did Institute, W. Va., narrowly escape a Bhopal-type catastrophe? (4/24)

A large explosion and fire that took the lives of two workers at the Bayer CropScience (Bayer) plant last August was caused by a thermal runaway reaction during the production of an insecticide, according to a U.S. Chemical Safety Board press release. The event likely resulted from significant lapses in chemical process safety management at the plant, CSB investigators said in preliminary findings released at a news conference.
