Soon after beginning their cleanup of a fume-filled tanker car at an Omaha, Neb., rail maintenance yard, Adrian LaPour and Dallas Foulk were dead. An explosion that April 2015 afternoon trapped LaPour in a flash fire inside the car and hurled Foulk out the top to his death.

After citations, Ill. contractor exposes same workers to same trenching hazards

Less than three weeks after being cited for exposing workers to unsafe trenches, federal investigators saw a Chicago plumbing contractor exposing the same four-man crew to trenching hazards as they worked on sewer and water utilities at two locations in Oak Park on consecutive days in March 2016.

Ammonia release shuts down Russell Stover Candies plant

The failure of an air-conditioning unit pipe released about 22 pounds of anhydrous ammonia into the air at the Russel Stover Candies Inc.’s Iola, Kansas facility on Sept. 23, 2015, setting off alarms and sending hundreds of workers scrambling for safety.

OSHA cites Sharpe Holdings in death of worker at Missouri dairy farm

Third fatality at company since 2012

For the third time since 2012, federal investigators have cited Sharpe Holdings in the death of an employee. The most recent casualty was a 51-year-old equipment operator, who suffered serious head injuries after he was ejected from the rear of a van on Sept. 26, 2015. He died the following day.

An employee working on the outside of a bridge abutment in a 12-15’ trench in Milllville, Pennsylvania died when the adjacent trench wall collapsed, burying him in soil. The worker was in the trench shoveling soil off the base of the abutment wall because it was not reachable by an excavator.

23-year-old worker loses part of finger in machine

Painesville, Ohio-based Dyson Corp. failed to use machine safety procedures

A 23-year-old machine operator making nuts and bolts suffered a partial amputation of his right middle finger when it was caught in a machine at a Painesville manufacturer. He had only three weeks' experience with the equipment when the injury occurred.

OSHA: Severe penalties possible for upstream drilling & well-servicing firms

In February OSHA announced that employers engaged in crude petroleum and natural gas extraction, drilling and related support activities are engaged in “high hazard” activities and will be subject to OSHA’s Severe Violator Enforcement Program.

Ohio company provides false proof of hazard abatement

After providing false documentation and making false representations claiming that previously cited hazards related to hydraulic presses had been corrected, Formed Fiber Technologies LLC has been issued 14 safety citations, including willful and repeat citations, as well as a notice of failure to abate with proposed fines totaling $816,500.

Explosion, fire injuries three workers at Ill. paint plant

Employee with clothing on fire had to crawl past blocked exit

Fox Valley Systems Inc. of Cary, Ill. has been cited for multiple safety violations following an explosion and fire that resulted in serious injuries to three employees on March 6 at the Cary plant. OSHA cited the company for 26 safety violations, including two willful violations where locked doors impeded exit routes and snow blocked exits, slowing employees from exiting the plant quickly.
