NTSB investigating fatal Hawaii skydivers plane crash

11 people killed in the incident

The engines on a plane full of skydivers sounded normal, according to a witness, but moments later, it crashed just after takeoff from a Hawaii airport, killing all 11 people aboard. The National Transportation Safety Board’s (NTSB) initial report into the June 21, 2019 incident provides no insight into possible causes of the crash. The NTSB investigation is ongoing.

Hawaii warehouse workers locked in

Company hit with 63 violations

A warehouse operator’s decision to seal exit doors and block emergency exit routes in order to gain additional storage space for hazardous materials “placed the workers in great jeopardy," said OSHA’s Galen Lemke, who pointed out that the blocked exits could have devastating results in the event of an ammonia leak from piping located throughout the facility.

Workers at a Hawaii hotel exposed to electrical, other hazards

OSHA has cited Halekulani Corp., owner and operator of Halekulani Hotel in Waikiki Beach, with 14 serious safety violations following a routine inspection. "The hotel industry is one of the biggest employers in Hawaii, and it's critical that owners and operators have strong safety and health programs," said Galen Lemke, director of OSHA's Honolulu Area Office.
