OSHA issued a fine of $75,156 to packaging firm Ampac Mobile Holdings LLC after two workers were injured by machinery at the firm’s manufacturing plant in Mobile, AL, the agency announced in a recent release. 

The company, operating under the name ProAmpac, was cited for failing to use machine guarding, neglecting to implement lockout/tagout procedures to control hazardous energy while maintenance or service work was being performed.

OSHA opened an investigation into the company under its National Emphasis Program on Amputations after a worker suffered severe injuries after their hand became caught in a piece of machinery and another worker experienced a finger laceration from moving machine parts. 

“A comprehensive safety and health program, to include an evaluation and correction for amputation hazards, could have identified and prevented these injuries,” OSHA Acting Mobile Area Office Director Jose Gonzalez said in a statement.
