A series of workplace incidents in Indiana and Illinois last week claimed the lives of two workers and left another hospitalized.

In Mooresville, Indiana, 22-year-old Sean Merrell died of blunt force injuries after being trapped in a machine he was cleaning that was re-energized before he could get out.

The Oct. 22 fatality occurred at PacMoore, a food manufacturing and processing plant.

A workplace incident at the Berner Foods & Beverage Plant in Dakota, Illinois, Oct 27 Illinois killed 45-year-old Marc Merriman. According to news sources, Merriman was operating a fork lift when he accidentally drove the vehicle off a loading dock.

Merriman was pronounced dead at the scene.

And in Geneva, Illinois, a female temp worker was injured on Oct. 25 when she was trapped in a conveyor belt assembly at Power Packaging, according to the Geneva Fire Department. The company’s maintenance staff was able to disassemble the machine and free her before first responders arrived. The victim was transported to an area trauma center for treatment.
