Worker falls 125 feet from cell tower

Custom Tower LLC of Scott, La., has been cited by OSHA for one willful safety violation following the death of a worker who fell approximately 125 feet while attempting to install a microwave dish on a cellular tower along Highway 149 in Louise. OSHA initiated the August inspection in response to the fatality.

Texas worker dies in accident while being trained

Company cited for 16 safety violations

OSHA has cited Coastal Plating Co. with 16 safety violations at its Corpus Christi facility. In May, an employee learning how to operate a blast cabinet was struck by a gas compressor power cylinder when it exited a cabinet. The worker died from his injuries.

Company owner sentenced to prison in employees’ deaths

Fatal explosion lead to manslaughter convictions
December 2, 2013

The owner of a New England munitions manufacturing company is heading to prison after being convicted on two counts of manslaughter in the deaths of two of his employees. Craig Sanborn was sentenced in Coös County Court in New Hampshire to five to ten years in prison in the May, 2010 explosion at his Black Mag LLC plant.

Contractors cited in Philadelphia building collapse that killed six

OSHA demolition standards violated
November 22, 2013

The fatal June, 2013 collapse of a four-story building in Philadelphia has resulted in OSHA violations against the contractors hired to demolish the building. Griffin Campbell, doing business as Campbell Construction, and Sean Benschop, doing business as S&R Contracting, were cited for three willful per-instance violations, following the incident, which killed six people and injured 14.

OSHA enforcement cases relating to hearing conservation

Earlier this year OSHA cited Kamps Inc. for 10 alleged safety and health -- including one willful -- violations at its Versailles wood pallet manufacturing facility. OSHA's inspection was initiated on Nov. 4, 2011, under the agency's Site-Specific Targeting Program, which expands the scope of a routine inspection to cover all hazards associated with the employer's industry. Proposed fines total $101,000.
