OSHA has cited food giant Pilgrim’s Pride Corp. with eight safety violations following the death of a worker who became caught in an unguarded hopper while attempting to remove a piece of cardboard at the company's facility in Canton, Ga.

OSHA has cited an Illinois contractor for seven safety violations, including three willful, for failing to protect workers from cave-ins and moving soil and chunks of asphalt during trenching operations. The inspection was initiated under OSHA's national emphasis program for trenching and excavation after an OSHA inspector witnessed apparent cave-in hazards while traveling past a construction site in Des Plaines on Oct. 3, 2012.
OSHA has cited Nuway Tobacco Co. for 22 alleged serious violations of workplace safety and health standards at its South Windsor manufacturing facility. The tobacco sheet manufacturer faces $59,869 in proposed fines following an inspection by the agency’s Hartford Area office.
OSHA is proposing $40,000 in fines for CVS Pharmacy Inc. for an alleged repeat violation of safety standards following an inspection of the retailer's Red Hook store by OSHA's Albany Area Office begun in January in response to a complaint.
Six months after an accident that resulted in a worker being paralyzed, OSHA has concluded its investigation into the incident and issued three citations against the employer, Ryan Roofing Inc. The worker is paralyzed after falling 20 feet from the roof of a commercial building the company was replacing on Oct. 3, 2012.
A natural gas company’s failure to provide a worker with FR clothing lead to a fatality, according to OSHA, which has cited Pennsylvania-based J.R. Resources for eight safety and health violations. The employee died from injuries sustained during a flash fire.
A gas well fire that injured three workers has resulted in two serious citations against a West Virginia drilling company. OSHA inspected Hall Drilling LLC’s facilities following the August 2012 incident as part of its regional emphasis program for the oil and gas industry. Proposed penalties total $12,600.
Despite previous violations, a tortilla manufacturer is still failing to provide employees exposed to corrosive chemicals with emergency eyewashes and is still not monitoring them for formaldehyde exposure.
Bacardi Bottling Corp. cited for safety violations

OSHA has cited Bacardi Bottling Corp. with 12 alleged safety violations following the death of a 21-year-old temporary worker his first day on the job. Lawrence Daquan "Day" Davis was crushed to death by a palletizer machine at the Jacksonville facility in August 2012.
Wyoming oil refineries faced more citations for dangerous safety conditions per oil barrel of production capacity than those in any other state over the past five years, a Casper Star-Tribune analysis of federal data shows. Government safety inspectors tagged Wyoming refiners with 239 citations for serious, willful or repeat safety violations in 2008-2012, according to data from federal OSHA. That means Wyoming refiners were cited on an average of every eight days.
