OSHA has cited SCR Construction Co. Inc. of Richmond, Texas with 17 safety violations and placed the company in its Severe Violator Enforcement Program following the death in January of a worker. The worker was killed when the flammable barrel he was torch cutting exploded at the employer's maintenance yard in Richmond.
Environmental Enterprises Inc. has been cited with 22 safety and health violations by OSHA stemming from a fire and explosion at the company’s Cincinnati waste treatment facility on Dec. 28 that killed one worker and left another with severe burns.

OSHA has cited an Illinois contractor for seven safety violations, including three willful, for failing to protect workers from cave-ins and moving soil and chunks of asphalt during trenching operations. The inspection was initiated under OSHA's national emphasis program for trenching and excavation after an OSHA inspector witnessed apparent cave-in hazards while traveling past a construction site in Des Plaines on Oct. 3, 2012.

Despite some problems, OSHA says its Severe Violator Enforcement Program (SVEP) is “off to a strong start.” That assessment came in a Severe Violator Enforcement Program White Paperissued this week by the agency.
OSHA has added Pandrol USA in Bridgeport, NJ to its Severe Violator Enforcement Program (SVEP) after the company was cited for 25 safety and health violations – including three willful. Proposed penalties total $283,500.
Companies that find themselves with membership in OSHA’s Severe Violator Enforcement Program (SVEP) now have clear guidelines for how they can get themselves removed from the program.
