Cal/OSHA has cited two companies $352,570 for multiple workplace safety and health violations, including ten serious and three willful category violations, following an incident in which a worker lowered into a 50-foot drainage shaft fell to his death. Neither D&D Construction Specialties, Inc. nor Tyler Development, Inc. followed permit required confined space procedures to work in confined spaces.

NYC construction owner charged with homicide in worker’s death

The owner of a New York City construction company has been charged with manslaughter and criminally negligent homicide in the death of an employee. News sources say Michael Weiss is accused of ignoring repeated requests for material to use in shoring up a retaining wall adjacent to a site where his employees were conducting excavation.

A contract worker was seriously injured when a trolley struck and crushed him inside the Wal-Mart Distribution Center in Brundidge, Ala. OSHA cited Wal-Mart and the worker’s employer, Swisslog Logistics Inc., for serious and willful violations for exposing workers to caught-between, struck-by and crushing hazards and for failing to implement lockout/tagout procedures.

The incident occurred just before 7:30 a.m. near the intersection of Lake Forest Boulevard and Michoud Boulevard, close to Interstate 10.

Fall fatalities among oil and gas extraction workers, 2005-2014

Previous research has shown that fatality rates for oil and gas extraction workers were decreasing for all causes of death except for those associated with falls. (1) A new study from National Institute for Occupational Safety and Health, published in the Morbidity and Mortality Weekly Report, examined risk factors for fatal fall events in this industry during 2005-2014 using data from case investigations conducted by the Occupational Safety and Health Administration (OSHA).
