Finger amputation leads to 21 citations against Mo. company

Machine not isolated from energy source

Piramal Glass USA Inc. has been cited by OSHA for 21 safety and health violations after a worker suffered a finger amputation while repairing a machine at the Park Hills manufacturing plant on March 14. Prior to maintenance, the machine was not isolated from its energy source. Proposed penalties total $137,400.

On farms, manure storage facilities are used to store animal waste, which can then be used for fertilizer. Farmers or farm workers may need to enter the facilities to repair or maintain equipment, such as pumps and intake hoses. These confined spaces are often oxygen-deficient atmospheres and can contain toxic and/or explosive gases that create a dangerous environment.

Worker fatality results in $1.33 million in criminal, civil fines against S.D. company

Adams Thermal Systems Inc. has entered into a deferred prosecution agreement with the U.S. Attorney's Office and OSHA to pay more than $1.33 million to resolve criminal penalties and OSHA fines levied as a result of the death of a worker on Nov. 7, 2011, in the company's Canton, S.D. plant.

Fire, explosions rock Oklahoma chemical plant

No employees at worksite at time of incident

A fire last night at a chemical plant in Thomas, Oklahoma, heated pressurized containers and caused a number of explosions at the facility, news sources are reporting. The chemical which burned was believed to be methanol, which is used as an antifreeze, solvent, fuel, and as a denaturant for ethanol and in the production of biodiesel.

Explosion, fire injuries three workers at Ill. paint plant

Employee with clothing on fire had to crawl past blocked exit
September 18, 2013

Fox Valley Systems Inc. of Cary, Ill. has been cited for multiple safety violations following an explosion and fire that resulted in serious injuries to three employees on March 6 at the Cary plant. OSHA cited the company for 26 safety violations, including two willful violations where locked doors impeded exit routes and snow blocked exits, slowing employees from exiting the plant quickly.
