A Federal Bureau of Investigation (FBI) study identified 200 active shooter incidents in the U.S. from 2000-2015 that killed or wounded a total of 1,274 people. A complaint of unsafe working conditions led OSHA inspectors to discover the safety and health of employees at a well-known Oklahoma truck bed fabricator being placed at risk amid nearly two dozen safety and health violations.
