A worker fell 200 feet to his death January 22 while replacing sound-absorbing tiles in the ceiling of Edward Jones Dome, home of the St. Louis Rams football team, in downtown St. Louis, Mo.

Michael Holt, 45, was part of a crew at the dome performing maintenance work. Holt fell from a beam, and was wearing a fall protection harness at the time, according to press reports. St. Louis police and OSHA are investigating the incident.
