Fatality case study involving pipe handling catwalk machine -- what happened?

A floor hand was caught between a pipe handling catwalk machine and a stanchion post on the drill floor of a dynamically positioned drillship. The operation at the time of the incident was picking up pipe from the pipe handling catwalk machine, making up at the rotary table, and running in the hole.

Bridge co. employee killed in Fla. bridge collapse

A bridge worker is among the six known fatalities of Thursday’s collapse of a pedestrian bridge in Florida. Navaro Brown, an employee of Structural Technologies VSL, has been identified as one of the people killed when a newly installed bridge intended to provide safe passage for students of Florida International University collapsed onto a busy highway, crushing a number of cars.

Fall kills Cirque du Soleil performer

A French aerialist plunged to his death Saturday night in Tampa, Florida, while performing an aerial straps routine in the famed Cirque du Soleil theatrical company’s “Volta” show. News reports say Yann Arnaud was performing on the double rings when one of his hands slipped and he fell about 20 feet to the stage in front of horrified spectators.

As many Americans continue to worry that the Supreme Court ruling on unions could change workplace safety laws for the worse, there remains a more general threat to workplace safety which U.S. businesses should be focused on. Despite a gradual decrease over the past 13 years, the workplace fatality rate per 100,000 people in the U.S. is still significantly higher than in most E.U. countries.

ASSE working on active shooter technical report

On the heels of an announcement by the National Fire Prevention Association (NFPA) that that organization is fast-tracking an active shooter response standard comes word that the American Society of Safety Engineers (ASSE) is also taking steps to address the issue. The ASSE convened a working group of safety and health experts to provide insights into the development of an active shooter technical report, which could guide organizations toward safer work environments with fewer hostile events.

Weekly Toll: Another group of workers join the list those who didn’t make it through the workday

Three school employees were among the 17 persons killed in the mass shooting at the Marjory Stoneman Douglas High School in Parkland, Florida. Scott Beigel, 35, a geography teacher, was killed as he tried to usher students back into his classroom when the shooting broke out. Aaron Feis, 37, an assistant football coach, was killed when he threw himself in front of students to protect them from oncoming bullets, according to football program spokeswoman Denis Lehtio.

Unshored trench collapses, kills Mich. worker

February 27, 2018
His fellow workers could hear his voice – at first – then a construction worker buried in a trench collapse fell silent, and died. The incident occurred at a suburban Detroit worksite at 1:30 yesterday afternoon – although emergency responders were not able to recover the man’s body for four hours, according to news reports. He was pronounced dead at the scene.

Ala. worker dies in a "preventable tragedy"

The death of a 45-year-old woman who was pulled into a plastics recycling machine has resulted in citations and penalties for an Alabama company. Emergency responders who arrived at ABC Polymer Industries LLC shortly after 6:41 p.m. on April 16, 2017 found that Eva Saenz of Alabaster, Alabama had sustained fatal injuries after being pulled into an industrial machine.

Man found dead, pinned underneath Bobcat in York County- PARADISE TWP., PA — A New Oxford man died Monday afternoon after being pinned underneath a piece of construction equipment. Shane Hockensmith, 30, was found unresponsive — and determined dead — under a Bobcat in the first block of Beaver Creek Road around 4:12 p.m., according to the York County Coroner’s Office.
