FRA announces cause of Mount Carbon derailment

The Federal Railroad Administration (FRA) today announced the cause of the February 16, 2015 CSX/Plains All American derailment in Mount Carbon, W.Va. The accident resulted in 27 derailed cars, a fire that ignited immediately and eventually burned for days and the evacuation of hundreds of local residents.

Winder Power, a UK company that manufactures transformers and generators, recently reached a milestone by recording 1,000 days without a reportable accident or lost-time incident. NTSB wants Amtrak to videotape its crew members

In a letter to Amtrak yesterday, the National Transportation Safety Board said that it should install crash- and fire-protected inward- and outward-facing audio and image recorders in the operating cabs of all of its trains, and review the recordings to ensure that crew actions are in accordance with procedures.
