Oil, gas “Rig Check” inspection forms available

December 21, 2012

NIOSH has released “Rig Check,” a collection of 35 inspection forms designed to be used by rig workers during the inspection of tools and equipment on oil and gas rigs. The monthly inspection forms cover emergency response, electrical safety, fall protection, stairways and walkways, lines and slings, tools, hoses, chemical hazards, ladders and platforms, and other equipment used on oil and gas rigs.

Company cited previously for hazard that killed worker

December 13, 2012

OSHA has cited ATW Automation Inc. for nine safety violations after a worker sustained blunt force trauma injuries at the company's machine manufacturing facility in Dayton, Ohio. The worker was caught and pinned by a conveyor that had lowered during a "power down" process, and he died from his injuries a few days later.

OSHA has cited Williams & Davis Boilers Inc. with nine safety violations - including one willful, four repeat and four serious - for continuing to expose workers to fall and other hazards at the company's facility in Hutchins. Proposed penalties total $131,670.

Follow up inspection at Wisconsin foundry doesn’t go well

Nine violations despite nine months to fix hazards

After a follow up inspection to determine if Grede Wisconsin Subsidiaries LLC abated the hazards found during the first go-round, OSHA has cited the company for nine – including two repeat – safety violations at its Berlin iron foundry. These carry $56,320 in proposed penalties.
