Disney fined for delays in reporting worker injuries

The Walt Disney Company has been fined $13,260 for failing to report two workers’ injuries in a timely manner. The injuries occurred at the company’s Epcot Center and Beach Club Resort in December 2018. One was serious enough that the employee required surgery.

"Knowingly and repeatedly" ignored fall protection requirements

OSHA has cited framing contractor Navy Contractors, Inc. for willfully exposing employees to fall hazards at residential construction sites in Royersford, Collegeville, and Center Valley, Pennsylvania. The company faces $603,850 in penalties. The agency initiated inspections at the three jobsites after inspectors saw employees performing framing work without fall protection.

Company managers indicted for obstructing OSHA investigation

Men threatened to fire workers who didn't change their stories
April 11, 2019
A federal grand jury in the Northern District of Ohio has indicted two managers at Extrudex Aluminum Inc. in Ohio for conspiracy to obstruct justice during a 2012 workplace fatality investigation by OSHA. The agency inspected the aluminum extrusion manufacturer after an employee suffered fatal injuries when a rack containing hot aluminum parts tipped over and pinned him. A second employee suffered severe burns.

27 violations found at Remington Arms manufacturing facility

April 8, 2019
OSHA has cited Remington Arms Company LLC – based in Madison, North Carolina – for 27 violations of workplace safety and health standards after an employee's fingertip was amputated while working on a broaching machine at its Ilion, New York, manufacturing plant. The arms manufacturer faces $210,132 in penalties.

Pallet maker continues to expose workers to wood dust

April 4, 2019
A Wisconsin pallet manufacturer has again been cited by OSHA after a follow-up inspection found employees continued to be exposed to wood dust. Avid Pallet Services LLC faces penalties of $188,302 for repeated, serious, and other-than-serious safety and health violations. Inspectors determined that the Beloit, Wisconsin, pallet manufacturer failed to implement sufficient engineering controls to limit dust exposure, as well as train employees on the health hazards of wood dust.

Worker killed by lightning; employer cited by OSHA

April 2, 2019
OSHA has cited C.W. Hendrix Farms Inc. for failing to protect workers from recognized hazards after lightning struck and killed an employee at the Parkland, Florida, agricultural operation. OSHA inspectors determined that Hendrix Farms exposed employees to lightning strikes as they picked vegetables in inclement weather. The company faces a penalty of $12,934, the maximum amount allowed.

No way out for workers in Georgia trench

OSHA: A trench can collapse "in a matter of seconds"
March 28, 2019
Employees of a Georgia contractor who were installing water and sewer lines had no safe means to enter and exit the excavation in which they were working, nor did the trench have a protective cave-in system. Those were among the excavation hazards OSHA investigators found at a Corley Contractors, Inc. worksite in Acworth, Georgia. The Dallas, Georgia-based company faces $106,078 in penalties.

Explosion kills 2 at Fla. ammo maker; company cited

March 25, 2019
OSHA has cited ammunition manufacturer AMTEC Less Lethal Systems Inc. for exposing employees to explosive hazards after an explosion fatally injured two workers at the company’s Perry, Florida, facility. The company faces $188,290 in penalties for multiple serious violations, and a willful violation that carries the maximum penalty allowed.
