OSHA has cited Hilti Inc. – a hardware merchant wholesaler – for exposing employees to struck-by hazards after an employee was injured while operating a forklift at a distribution center in Atlanta, Georgia. The Plano, Texas-based company faces penalties of $164,802.

Workplace amputation leads to OSHA fines for Texas food company

March 1, 2019
OSHA has cited Bruce Foods Corporation – a Mexican food manufacturer based in El Paso, Texas – after an employee suffered an amputation. The company now faces $194,350 in fines. OSHA cited the company for 24 serious safety violations including failing to train employees in lockout/tagout procedures, inadequate machine guarding, lack of fall protection, and exposing employees to live electrical parts.

Only $9,472 for a head injury that put TV stunt man in a coma

February 27, 2019
A motion picture company has been issued a serious citation by OSHA for an accident in August 2018 that left a stunt man hospitalized for weeks with serious injuries. The agency says Eye Productions, Inc. failed to protect stunt coordinator Justin Sundquist from hazards while filming the CBS television show MacGuyver in Chattahoochee Hills, Georgia. Sunquist fell from the back of a vehicle that was traveling at approximately 18 mph, according to OSHA.

OSHA investigators found that employees at a Texas gun range were not only exposed to above-permissible limits of lead in the air, they also risked potential exposure due to surfaces throughout the facility that were contaminated with the substance. Tap Rack Bang Indoor Shooting Range LLC - operating as The Gun Range faces penalties totaling $214,387 for safety violations at its facility in Killeen.

January 21, 2019
Kansas drywall contractor cited following fatal forklift incident. Midwest Drywall Company, Inc., faces $77,604 in penalties after a worker was fatally crushed by part of a forklift. OSHA inspectors determined that the company failed to properly secure a suspended and supported load, and ensure that workers were kept clear of it.

OSHA penalties adjusting in 2019

January 18, 2019
OSHA's civil penalties amounts for violations of workplace safety and health standards will increase in 2019 to adjust for inflation. The adjusted maximum penalty amounts will take effect upon publication in the Federal Register. New penalties for willful and repeat violations will be $132,598 per violation; serious, other-than-serious, and posting requirements are $13,260 per violation.

Worker dies from gas exposure in trench

January 18, 2019
OSHA has cited RKM Utility Services Inc. for failing to protect workers from hydrogen sulfide after an employee died after exposure to dangerous levels of the gas while working in a trench in Dallas, Texas. OSHA inspectors determined that the company exposed employees to a hazardous atmosphere, failed to train employees on the health hazards of hydrogen sulfide, and did not drain water from the trench.

January 16, 2019
A volunteer firefighter in Sun Prairie, Wisconsin, lost his life last July responding to an explosion and fire that resulted from two utility contractors failing to establish the location of underground utilities prior to beginning excavation work.

Trench was filling with water, had no cave-in protection
January 15, 2019
OSHA has cited Spear Excavating LLC – based in Pennsburg, Pennsylvania – for exposing employees to trenching hazards at a worksite in Malvern, Pennsylvania. The company faces $106,057 in proposed penalties. OSHA initiated an inspection on August 2, 2018, after receiving a complaint alleging the hazards.
