Wisc. temp worker suffers electrical shock burns

December 7, 2014

A 24-year-old temporary maintenance employee sustained severe burns from electrical shock while on assignment for Parallel Employment Group of Wisconsin Inc. Working at the Arvato Digital Services LLC distribution center in Pleasant Prairie, Wisc., the employee came in contact with an energized electrical source and suffered electrical shock.

Worker fatally injured at Milwaukee concrete plant

Another worker exposed to same hazard at same plant the next day
December 5, 2014

A 39-year-old worker and father of three was crushed to death on May 22, 2014, while repairing a bin at the Ready Mix Batch Plant owned by Nuvo Construction Co. in Milwaukee. The following day, an employee for a different company, Sonag Ready Mix LLC, completed repairs to a bin sensor at the Ready Mix Batch Plant while exposed to the same hazard.

Freight workers exposed to electrocution, fall, crushing hazards

Central Transport LLC faces $330,800 in fines; cited elsewhere for similar hazards
December 4, 2014

Employees at the Central Transport LLC freight shipping terminal in Billerica, Massachusetts were exposed to electrocution, falls, crushing and other injuries due to their employer's knowing and repeated disregard for basic worker safeguards, OSHA has found. The company faces $330,800 in fines for these hazards.

Texas stone fabricator faulted for crushing hazards

December 1, 2014

Berry Marble and Granite exposed workers to crushing hazards associated with handling granite, marble and stone at its Tyler, Texas, facility, OSHA found in a follow-up inspection. The company has received 14 citations, including two for willfully violating safety standards and two for repeated violations, with a proposed fine of $156,310.

Philly bricklayers exposed to 30 feet falls at residential construction sites

T and S Masonry fined $100,560
November 20, 2014

Bricklayers were exposed to fall hazards as high as 30 feet above the ground while creating the exterior finish of new single-family dwellings at two work sites in Philadelphia, according to workplace safety and health citations issued against Vyacheslav Leshko, doing business as T and S Masonry LLC.

Penda Corp. cited for 7 violations
November 17, 2014

Workers cleaning a chemical spill at Penda Corp. in Portage had not been trained in proper cleanup procedures or provided proper personal protective equipment, according to an investigation by the U.S. Department of Labor's Occupational Safety and Health Administration.

Machine operator fatally crushed at Pa. manufacturing facility

November 13, 2014

A 45-year-old machine operator was fatally crushed while he adjusted a set screw on a rolling mill at Cleveland Track Material on Aug. 21, 2014. An OSHA investigation found that workers were exposed to dangerous rotating machinery parts at the facility that rolls steel for railroad industry use. Seven serious violations have been issued as a result of the investigation.

Wisconsin manufacturing company cited in death of worker

November 10, 2014

A 55-year-old worker who was found unresponsive in a restroom at Bremer Manufacturing Co. Inc. on May 6, 2014 later died. The man, who had been employed at the Elkhart Lake foundry for the past 38 years, worked with resin-containing isocyanates on the sand molding line.

In Florida: 17 feet up and no fall protection

More than $70K in penalties proposed by OSHA
November 7, 2014

Residential framing and siding contractor Southern Wood Works LLC has been cited by OSHA for five safety violations following an August 2014 inspection at a residence in Watersound. Citations were issued to the Freeport, Fla.-based company for failing to provide employees with fall protection equipment and exposure to falls from heights of up to 17 feet.
