Employee fatally injured after being caught in machinery at Chicago bakery

A 31-year-old engineer was fatally injured on May 27, 2014, when his head was struck by an unguarded rotating gear arm on a piece of bakery equipment at Alpha Baking Co. Inc. OSHA has cited the Chicago baking plant for six serious safety violations following the tragic incident.

Georgia trash hauler failed to provide vaccine to employees

October 28, 2014

Stafford Transport Inc., a solid waste removal trucking company, has been cited by OSHA for five safety and health violations following an inspection at the company's Mableton facility. OSHA initiated the inspection in July 2014 as part of the agency's Site-Specific Targeting Program, which directs enforcement resources to workplaces with higher-than-average rates of injuries and illnesses.

Three fires, eight citations

Reports of three separate fires at a Wisconsin coatings company led to an OSHA inspection – and the issuance of citations for eight safety violations. Many of the violations against Quest Specialty Coatings were cited under OSHA’s Process Safety Management Standards, which contain specific requirements for managing highly hazardous chemicals in work processes.

An employee was severely injured when he became caught in unguarded arbor rollers at Kloeckner Metals Corp. on April 22, 2014. OSHA has cited the Cincinnati manufacturing plant with one repeat and three serious safety violations, carrying proposed penalties of $56,000.

Five violations issued in deadly Florida tree trimming accident

October 22, 2014

A 39-year-old tree trimmer was killed when the mechanical arm supporting the bucket of an aerial lift suddenly collapsed, striking the cab of the truck and ejecting the worker onto the pavement. As a result of the fatality, OSHA cited Advanced Tree Service in Newberry for five safety violations.
