Manufacturing employee injured while doing machine maintenance

OSHA has cited Supplyside USA of Lenox, Illinois for machine safety violations after an employee was injured while conducting maintenance on equipment. The pallet manufacturer faces $91,832 in proposed penalties for two repeated, six serious, and three other-than-serious violations.

Hand crushed, fingers amputated at Ohio manufacturing company

January 18, 2018
Serious injuries to two employees brought OSHA inspectors to a vinyl floor manufacturing facility in Fostoria, Ohio, where they found multiple safety violations. Nox US LLC, an Ohio manufacturer of luxury vinyl tile, now faces $514,236 in proposed penalties and has been placed in OSHA’s Severe Violator Enforcement Program. Both injuries occurred in June 2017.

Worker injuries reveal hazards at auto parts plant

November 8, 2017
Two serious injuries to workers – including an amputation of a left index finger -- brought OSHA investigators to an auto parts manufacturing facility in Buford, Georgia. The agency subsequently cited Elringklinger USA Inc. for exposing workers to electrical, fall, and noise hazards. Proposed penalties total $308,906. OSHA inspected Elringklinger USA on May 2, 2017, after an employee performing maintenance on a screen print machine was injured.
