CSB: Utah refinery blast caused by corrosion, lack of inspection

Massive release of hydrogen sent a blast wave through residential area

A massive explosion and fire at the Silver Eagle Refinery on November 4, 2009, in Woods Cross, Utah, which damaged homes in a nearby neighborhood, was caused by a rupture in a pipe that had become dangerously thin from corrosion, according to the U.S. Chemical Safety Board (CSB).

NTSB: Natural gas pipeline rupture, fire caused by corrosion

Pipeline not inspected since 1988

The 2012 rupture of a Columbia Gas Transmission Corporation natural gas pipeline in West Virginia was caused by external corrosion that could have been discovered by the pipeline operator, the National Transportation Safety Board said in a report released earlier this week.

BP again allowed to do business with federal government

EPA lifts suspension and debarment of company
March 14, 2014

The Environmental Protection Agency and BP yesterday executed an agreement resolving all suspension and debarment actions against BP that barred the company from doing business with the federal government following the company’s guilty plea in the Deepwater Horizon disaster of April 2010. The administrative agreement will be in place for five years.

California moves ahead on refinery safety improvements

California’s initiative to improve refinery safety is getting a big thumb’s up from the U.S. Chemical Safety Board (CSB). A report recently released by the California Interagency Working Group on Refinery Safety “is an important milestone for improving refinery safety across the State of California,” said CSB Chairperson Rafael Moure-Eraso.

The U.S. Chemical Safety Board (CSB) is demanding access to the Tesoro refinery in Martinez, California, in order to investigate a Feb. 12 chemical spill that resulted in first- and second-degree burns to two workers.
