A worker who sought emergency treatment for leg burns due to an acid spill alerted OSHA to a lack of PPE at a company that uses acid, hazardous chemicals, lead and cadmium in its Illinois manufacturing facility. The company, Advanced Strobe Products, manufactures strobe and other lights for the aerospace, photographic, vehicular, warning, beacon, and machine industries.

OSHA has cited Phoenix Industrial Cleaning for 28 serious safety violations following the death of a worker who fell from ladder inside of a storage tank, apparently after being overcome by methylene chloride vapors at a chemical manufacturing facility in Wheeling, Ill. on Nov. 29, 2012.

OSHA has cited Rothe Welding Inc. of Saugerties for alleged failure to abate and repeat and serious workplace health violations, some of which refer to a previous 2012 OSHA inspection.
