OSHA has cited four New Jersey contractors working on a 20-story building in Jersey City for exposing workers to fall hazards following a December 2011 inspection during which inspectors observed employees working on the fourth floor without personal fall protection or fall protection systems.

OSHA has cited aluminum castings producer Trio Foundry Inc., which operates as Sandwich Castings and Machine, with 20 health and safety violations, including one willful and one repeat violation.

OSHA has cited Quail International Inc. with 23 safety and health violations at the company's Greensboro plant following an inspection that was initiated in January based on a complaint. Proposed penalties total $92,115.
 

The U.S. Department of Labor's Mine Safety and Health Administration (MSHA) has proposed $594,100 in fines to Manalapan Mining Co. Inc.'s P-1 Mine in Harlan County, Ky., for four violations. The proposed penalties were assessed as a result of an investigation into the June 2011 death of a miner who was fatally injured when a large section of rock fell from the underground coal mine's wall, or rib, and knocked him into a dolly.

OSHA  has cited Tribe Mediterranean Foods, a subsidiary of Nestle SA, that manufactures Tribe brand hummus products, for 18 alleged violations of workplace safety standards following the death of a worker at its Taunton production plant.

OSHA has cited Ohio truck manufacturer Stahl/Scott Fetzer Co. for six safety violations – including one willful – after a worker had several fingers amputated while operating an unguarded press break March 19 at the Wooster plant.

OSHA has cited Illinois Gun Works Ltd. for 28 alleged health violations following a Jan. 21 inspection referred by the Illinois Department of Public Health that found two gun range operators were exposed to airborne lead levels up to 12 times the permissible level. Proposed fines total $111,000.
