Inadequate shoring may have caused the roof collapse yesterday in Houston that injured nine construction workers at a hotel being built in north Houston. According to news sources, the incident occurred while workers were pouring concrete.

NJ med center loses appeal over OSHA citations

July 19, 2019
The 2016 case of a worker killed by an electric shock while repairing a ceiling light fixture came to a close recently, with an administrative law judge affirming OSHA citations issued to the man’s employer. The fatality involved a worker who was untrained in electrical safety work practices. The Occupational Safety and Health Review Commission (OSHRC) judge ruled that Jersey City Medical Center willfully failed to train the employee for the hazardous electrical work he was directed to perform.

Staffing firm again cited for safety lapses after young temp worker loses fingers

Last October, Erick Solis, a 19-year-old temp worker at a Los Angeles food company, lost two fingers when his hand got caught in an unguarded dough-rolling machine. Cal/OSHA, the state job safety agency, cited the company, JSL Foods Inc., for willful violations because an almost identical accident had happened before.

Worker falls into 10-foot hole near Disney World parking lot

A construction worker was trapped for at least 90 minutes after a 10-foot-deep hole opened up outside Disney's Animal Kingdom on Tuesday, according to Orange County Fire Rescue. Firefighters responded just before 2 p.m. to a construction area next to the rideshare pickup and drop-off area in the parking lot.

Recent high school grad killed in warehouse accident

July 12, 2019
A teenager who graduated from high school last month was killed July 2nd in a warehouse incident in Indiana. News sources say 18-year-old Timothy “TJ” Rich Jr. died at an Aldi warehouse in Greenwood. Rich was loading a truck when a dock plate – a device used to bridge the gap between a truck and the warehouse floor – came down, killing the teen.

Zookeeper injured by rhino; zoo cited by OSHA

June 26, 2019
OSHA has cited the Jacksonville Zoo in Florida for an incident earlier this year in which a zookeeper was seriously injured by a rhinoceros. News reports say Archie, a 50-year-old male Southern white rhinoceros, struck the zookeeper with his horn. She was treated for her injuries at a local hospital and released after several days.

Texas tire facility cited after worker fatality

OSHA has cited Southern Tire Mart LLC – based in Columbia, Mississippi – for failing to protect employees from serious safety hazards after a worker suffered fatal injuries while attempting to mount a monster truck tire rim at the company’s retreading facility in Fort Worth, Texas. The company faces $341,195 in fines.

Young carnival worker suffers life-changing injury

With the summer festival and fair season barely underway, a young worker in Michigan suffered an injury so serious on Saturday that he had to have a leg amputated. News sources say the 22-year-old was critically injured at the Curwood Festival in Owosso, a small community 94 miles northwest of Detroit.

Florida construction fall reveals a host of hazards

OSHA inspectors who arrived at a Florida construction site to investigate an employee’s near-fatal fall didn’t have to look far to find fall and other safety hazards at the project. Three of the four Florida-based residential contractors involved with the project earned citations for fall hazard-related violations. The four companies were cited for a total of 12 violations, with $220,114 in proposed penalties.

Five years of legal wrangling following a workplace amputation – in which retaliation, intrigue and secret photos played a part – ended recently with a decision by a federal jury in Pennsylvania. The jury in the U.S. District Court for the Eastern District of Pennsylvania found that Lloyd Industries Inc. and its owner, William P. Lloyd, unlawfully fired two employees because of their involvement in an OSHA investigation.
