Temp worker loses fingers at manufacturing plant

Cal/OSHA has issued more than $300,000 in serious citations to two employers after a temporary worker lost two fingers cleaning machinery at a food manufacturing facility in Los Angeles. The worker was cleaning a dough rolling machine when his left hand was partially pulled into the moving rollers and two of his fingers were amputated.

Disney fined for delays in reporting worker injuries

The Walt Disney Company has been fined $13,260 for failing to report two workers’ injuries in a timely manner. The injuries occurred at the company’s Epcot Center and Beach Club Resort in December 2018. One was serious enough that the employee required surgery.

Fla. worker injured in 26’ fall; company cited

"Providing workers with fall protection is not optional"

An Ohio-based construction company failed to ensure that workers used fall protection while doing roofing tasks, leading to a worker sustaining a serious injury, according to an OSHA inspection following the incident. The agency has cited Hiebert Bros Construction LLC for exposing employees to fall hazards after the worker was injured from a 26-foot fall at a construction worksite in Gainesville, Florida.

NTSB: CSX workers were walking on tracks when struck, killed

April 10, 2019

The 2017 deaths of two CSX Transportation workers were the result of their decision to walk on an active Amtrak train track in Washington, D.C., the National Transportation Safety Board (NTSB) said in a report issued Tuesday. Investigators found that while the conductors were likely aware that the two tracks were active, they may have chosen this more comfortable way to walk back to the front of their train because no Amtrak trains had passed through the area for about an hour.

Roadside accidents in Mich., Ohio leaves construction workers dead, injured

April 5, 2019
A Michigan man is hospitalized with critical injuries after he was struck by a co-worker who was backing up in a construction zone. News sources say Daniel Clark Jr. has collapsed lungs, broken ribs, a fractured pelvis and other injuries. The incident occurred Wednesday morning along I-75, in Troy, a suburb of Detroit. Troy police responded to the accident at around 9:30 a.m. Clark had reportedly started working for the company on Monday.

Explosion kills 2 at Fla. ammo maker; company cited

March 25, 2019
OSHA has cited ammunition manufacturer AMTEC Less Lethal Systems Inc. for exposing employees to explosive hazards after an explosion fatally injured two workers at the company’s Perry, Florida, facility. The company faces $188,290 in penalties for multiple serious violations, and a willful violation that carries the maximum penalty allowed.

Michigan man dies after falling into tank of sulfuric acid

March 7, 2019
A 54-year-old South Lyon man died after he fell into a tank of sulfuric acid. The incident occurred around noon at the Michigan Seamless Tube and Pipe facility. The cause of the man's fall remains unknown. After being pulled from the tank by a co-worker, the man was put into a shower and then rushed to The University of Michigan Hospital in Ann Arbor where he died Saturday night. The 160-degree acid is used to strengthen the piping manufactured at the plant.

Worker crushed by beam at San Francisco tunnel project

March 5, 2019
The employers of a worker killed on a light rail tunnel project in San Francisco failed to identify potential hazards and to train workers on safety procedures, according to Cal/OSHA, which has issued $65,300 in penalties in the fatality. The incident occurred last August, while employees were using heavy equipment and tools to work in and around the tunnel.

No forklift training for injured distribution center employee

March 4, 2019
OSHA has cited Hilti Inc. – a hardware merchant wholesaler – for exposing employees to struck-by hazards after an employee was injured while operating a forklift at a distribution center in Atlanta, Georgia. The Plano, Texas-based company faces penalties of $164,802.
