SUV crashes into disabled bus, kills tow truck driver

A fatal accident yesterday in southeast Michigan claimed the life of a tow truck driver and sent five people to the hospital with serious injuries. According to a news release from the Michigan State Police (MSP), the two truck driver was servicing a disabled school bus that was stopped on the side of a freeway when the accident occurred at about 10:20 a.m.

Extreme weather led to chem plant fire, hazmat release

May 29, 2018
Excessive rain caused by Hurricane Harvey was a key factor in the fire and subsequent hazmat release at the Arkema chemical plant in Crosby, Texas, according to the U.S. Chemical Safety Board (CSB), which has released its final investigation report into the August 31, 2017 incident. The report notes that in the days leading up to the incident, an “unprecedented amount” of rain fell at the plant due to Hurricane Harvey, causing equipment to flood and fail.

NTSB studying cracks in Miami pedestrian bridge that collapsed

May 23, 2018
Cracks in the north and south ends of the pedestrian bridge that collapsed in Miami March 15th are being scrutinized by the National Transportation Safety Board (NTSB) in its ongoing investigation of the incident. One bridge worker and five other people - occupants of crushed cars - were killed when the 174-foot-long bridge fell onto an eight-lane street below.

Calif. Conservation Corps failed to heed warning signs before fatal van crash

Early on Feb. 2, 2016, a van carrying members of the California Conservation Corps paused at a stop sign on a country road near the Central Valley town of Reedley. Then the van rolled into the intersection, where it was broadsided by a 40-ton gravel truck and trailer, killing three corps members and leaving another with catastrophic brain and spinal injuries.

Flammable gas leaked before Texas chem plant blast

May 22, 2018
Eighteen of the 21 workers injured in an explosion at a Texas chemical plant on Saturday have been released from hospitals after being treated for burn injuries and injuries from falling off a scaffold, according to news sources. Both the U.S. Chemical Safety Board (CSB) and OSHA are investigating the incident at the Kuraray America facility in Pasadena, which may have been caused by a leak of ethylene, a highly flammable gas used in the manufacture of plastics.

Plane crashes after losing left wing

May 16, 2018
Fatigue may have been behind the fatal April 4 crash in Florida that killed two people – but not pilot fatigue. An investigative update issued by the National Transportation Safety Board revealed that “fracture features” consistent with metal fatigue were found on more than 80 percent of the lower spar cap and portions of the forward and aft spar web doublers of the left wing main spar on the Piper PA-28R-201.

An OSHA investigation into a fire at a New York State manufacturing facility that claimed the life of an employee uncovered a host of safety hazards – for which the company is facing proposed fines of $281,220. New Windsor-based Verla International LTD was cited by the agency for failing to protect its workers from dangerous chemicals, and other hazards.
