50th anniversary of fatal train crash – and PTC still elusive

If a federal agency can be frustrated, the National Transportation Safety Board (NTSB) is frustrated. The NTSB is commemorating a five decades old railroad tragedy today, and pointing out that the safety recommendation it made in the wake of that incident remains largely unadopted, mostly due to Congressional interference. Feds propose changes to regulations limiting truckers’ hours

August 19, 2019

The Federal Motor Carrier Safety Administration (FMCSA) has unveiled five proposed changes to existing hours of service (HOS) rules for commercial motor vehicle drivers. Predictably, the revisions – which FMCSA says will increase safety and save money – are drawing mixed reactions. U.S. Transportation Secretary Elaine L. Chao said the changes would give commercial drivers more flexibility while maintaining the safety limits on driving time.

Gas line rupture claims a life in Kentucky

August 1, 2019
A fire caused by a gas line rupture early this morning in Kentucky killed one person and injured at least others, according to news reports. The fire damaged a half dozen mobile homes as well as train tracks. Don Gilliam, director of Lincoln County Emergency Management, said the 30” gas line breached shortly before two a.m in Hustonville.

Beware of this risk in pools, marinas

With plenty of summer still ahead, the National Fire Protection Association (NFPA) is reminding people about potential electrical hazards that exist in swimming pools, hot tubs and spas, onboard boats and in waters surrounding boats, marinas, and launch ramps. Most people have never heard of nor are they aware of electrical dangers posed in water environments such as electric shock drowning (ESD).

Inadequate risk assessments led to fatal SC train crash

The National Transportation Safety Board (NTSB) determined Tuesday that Amtrak’s and CSX Transportation’s failure to properly assess and mitigate the risk of conducting switching operations during a signal suspension, coupled with a CSX conductor’s error, led to the collision of an Amtrak train with a CSX train near Cayce, South Carolina.

FAA expands opportunities for safe drone operations

Even as it adds to its list of “no-fly zones” for drone operators, the Federal Aviation Administration (FAA) is taking steps to expand its Low Altitude Authorization and Capability (LAANC) system to include recreational flyers. This action – which begins today - will significantly increase the ability of drone pilots to gain access to controlled airspace nationwide.

NTSB closes 8 safety recommendations

"Much more work remains to be done"
July 17, 2019
The National Transportation Safety Board (NTSB) this week published an updated list of the safety recommendations associated with the agency’s 2019 – 2020 Most Wanted List of Transportation Safety Improvements following the recent closure of eight safety recommendations. Of the eight closed safety recommendations; four (P-17-003, H-15-020, A-09-092, and H-09-018) were closed with acceptable action taken, one (P-18-003 ) was closed with acceptable alternate action taken, one (M-16-028) was closed with a status of exceeds recommended action, and safety recommendation H-12-029 was unfortunately closed with unacceptable action taken.

Ten killed, plane destroyed in post-takeoff crash, fire

July 15, 2019
An airplane that crashed right after takeoff in Addison, Texas last month seemed to lack a normal power level as it taxied down the runway, according to the National Transportation Safety Board (NTSB) investigation into the incident, which claimed the lives of ten people. On the morning of June 30, 2019, the plane – which was bound for St. Petersburg, Florida - collided with a hangar and terrain after takeoff from Addison Airport (KADS).

EPA says existing regulations sufficient for electric power facilities hazmat risk

July 5, 2019
After evaluating the risk from electric power generation, transmission and distribution facilities, the EPA says no additional federal requirements are needed to cover the costs of possible hazardous substance releases. Using the authority of the Comprehensive Environmental Response, Compensation, and Liability Act (CERCLA) Section 108(b), EPA analyzed the need for financial responsibility requirements for the electric power industry.
