Eight U.S. miners killed in accidents in third quarter of 2014

About every other day during the past decade, a gas leak in the United States has destroyed property, hurt someone or killed someone, a USA TODAY Network investigation finds. The most destructive blasts have killed at least 135 people, injured 600 and caused $2 billion in damages since 2004.

About 30 old propane storage tanks sitting out on a property fronting State Highway 274 in Seven Points, TX, were tampered with October 13 in an apparent attempt to empty them and remove their regulators for scrap, according to The Monitor Online news agency.

Reports of three separate fires at a Wisconsin coatings company led to an OSHA inspection – and the issuance of citations for eight safety violations. Many of the violations against Quest Specialty Coatings were cited under OSHA’s Process Safety Management Standards, which contain specific requirements for managing highly hazardous chemicals in work processes.

The U.S. Department of Labor’s Mine Safety and Health Administration has fined Star Mine Operations, LLC $1,077,800 following its investigation into the deaths of two miners at Revenue Mine on Nov. 17, 2013. The underground silver ore mine is located in Ouray County, Colorado.

An employee was severely injured when he became caught in unguarded arbor rollers at Kloeckner Metals Corp. on April 22, 2014. OSHA has cited the Cincinnati manufacturing plant with one repeat and three serious safety violations, carrying proposed penalties of $56,000.
