A West Virginia company faces $117,500 in fines for failing to protect its workers from safety and health hazards from a job site in Cheshire, Ohio, according to OSHA.

Onyx Industrial Services, Inc., of Nitro, W.Va., was cited for multiple violations after OSHA investigations found two Onyx employees engulfed in lime slurry while inside a tank at the American Electric Power generating station in Cheshire. The power company hired Onyx to perform maintenance.

Among the citations issued to the company was a willful citation for failing to implement confined space entry conditions, exposing employees to engulfment and entrapment hazards.

Alleged serious violations included:
  • failing to provide adequate personal protective equipment;
  • failing to provide training on the hazardous conditions of entering a confined space; and
  • failure to identify rescue and emergency services for workers.
