Iowa worker’s arm amputated by grinding machine; company cited

An Iowa tire recycling company has been cited by the state’s Occupational Safety and Health Association (Iowa OSHA) after one of its employees suffered a partial amputation of his arm while on the job. News sources say the incident at Liberty Tire Recycling in Des Moines occurred on May 9, while workers were performing maintenance on the conveyor system of a tire grinding machine.

Zookeeper injured by rhino; zoo cited by OSHA

OSHA has cited the Jacksonville Zoo in Florida for an incident earlier this year in which a zookeeper was seriously injured by a rhinoceros. News reports say Archie, a 50-year-old male Southern white rhinoceros, struck the zookeeper with his horn. She was treated for her injuries at a local hospital and released after several days.

California OSHA issued four citations and $68,438 in penalties to USF Reddaway Inc. after a worker was fatally struck by a tractor at a truck terminal. Inspectors found that the company failed to ensure operators were competent to operate terminal tractors and did not implement traffic controls. Appeals court denies review of fatal electrocution

June 21, 2019
A federal appeals court on Tuesday denied an appeal requested by a home contractor facing a serious U.S. Occupational Safety and Health Administration citation following the electrocution of two workers, one of whom died of his injuries. In 2016, on a residential construction site in Alpharetta, Georgia, a subcontractor employee supervised by Century Communities Inc. was operating a crane within 20 feet of live overhead power lines, resulting in an electrical arc flash that caused the injuries and fatality.
